Dementia Friendly Canada

Dementia-Friendly Canada project is a partnership between the Alzheimer Societies of Canada, British Columbia, Saskatchewan, Manitoba and Ontario and is intended to foster the creation of dementia-friendly communities across the country.
A dementia-friendly community is a place where people living with dementia, their families and care partners feel included and supported. While creating dementia-friendly communities, individuals and organizations should focus on both the physical and social environments to ensure they are fully accessible.

The Ontario Caregiver Organization (OCO) exists to support Ontario’s 4 million caregivers; ordinary people who provide physical and emotional support to a family member, partner, friend or neighbor. We support caregivers by being their one point of access to information, so they have what they need to be successful in their role.

The Toronto Seniors Helpline is a single point of access for seniors and caregivers to receive information and access to community, home, and crisis services.
Toronto Seniors Helpline is funded through the Toronto Central Local Health Integration Network (LHIN), in partnership with WoodGreen Community Services. Toronto Seniors Helpline integrates the services of the former Seniors’ Crisis Line and the Community Navigation Access Program (CNAP) Hub. Seniors and caregivers only need to call one phone number in order to access information and referrals for community and homecare services.

Home Health and Community Support

Home care serves people of all ages with a wide range of health problems and disabilities at home, school, and in the community. Home health care services may be purchased directly, covered by private insurance plans, or publicly funded. In Ontario, Home and Community Care Support Services handle publicly-funded home health care. Assisted living programs provide support, including equipment and medical supplies, so that people can live independently in their own homes for as long as possible.
